Samsung Health Expands AI-Powered Diabetes Prevention and Chronic Condition Support for Older Adults Through Lark Health

10/26/2026

Samsung Electronics America today announced a collaboration with Lark Health to expand access to AI-powered prevention and chronic-care support for older adults (age 65 and older) through Samsung Health.

The collaboration begins with Lark’s Medicare Diabetes Prevention Program (MDPP) for eligible users and represents the first phase of a broader rollout that is planned to include technology-supported chronic care through the Centers for Medicare & Medicaid Services (CMS) Innovation Center’s Advancing Chronic Care with Effective, Scalable Solutions—or ACCESS—Model. Through ACCESS, eligible Samsung Health users may gain access to additional Lark programs supporting cardiometabolic and behavioral-health conditions, combining AI-powered coaching with clinical oversight and ongoing health tracking and clinical resources at no additional out-of-pocket cost.

Designed with top Harvard and Stanford behavior change scientists and clinicians, each Lark program provides 24/7 AI-powered coaching1 and health tracking through a smartphone, helping users build sustainable habits related to nutrition, physical activity, sleep and stress. Lark’s personalized guidance, interactive lessons and real-time feedback have helped 2.5 million patients incorporate healthier choices into their everyday lives.

Lark is a large CDC Diabetes Prevention Program Provider in the U.S. and an official Medicare Diabetes Prevention Program (MDPP) provider under guidelines from CMS. It is one of the first programs that will be available to Samsung users through the partnership. It is available to individuals who meet applicable Medicare eligibility and qualification requirements, including having Medicare Part B and not having an active diabetes diagnosis. Beyond A.I. powered coaching, eligible members receive live clinical resources and healthcare tools such as a cellular scale.

Samsung Health users age 65 and older who do not qualify for MDPP may instead be offered Lark Prevention; a separate one-year prevention program available at no additional out-of-pocket cost (a $249 annual value).2 This additional pathway allows more users to access digital coaching and tools that support healthier daily habits, even if they do not meet the eligibility requirements for the Medicare program.

Diabetes is highly prevalent among older adults, with approximately 38.4 million people in the U.S. living with the condition and the prevalence increasing with age. Roughly 33% of adults aged 65 or older (about 1 in 3) have diabetes, most commonly type 2 diabetes, which accounts for 90–95% of cases in this age group. Pre-diabetes is also estimated to affect 115 million people in the U.S. and hypertension is estimated to affect 119 million. 3,4,5,6

“With an aging population and chronic conditions on the rise, care has to move beyond the clinic and into people’s everyday lives,” said Dr. Hon Pak, Senior Vice President and Head of the Digital Health Team, Mobile eXperience Business at Samsung Electronics. “Our collaboration with Lark is a concrete step forward for Samsung’s Connected Care vision — starting with diabetes prevention today and expanding toward technology-supported chronic care that helps more older adults get the support they need, when and where they need it.”

This phased rollout reflects Samsung’s broader effort to connect everyday health insights with accessible services that can help people take action. By bringing prevention and chronic-care resources into the Samsung Health experience, Samsung aims to make it easier for users to discover relevant support, understand their options and connect with programs that meet their individual health needs.

Samsung Health was recently accepted into the Medicare App Library, a centralized directory that helps Medicare beneficiaries discover and connect with digital health tools and mobile applications. Samsung’s “Kill the Clipboard” initiative—a collaboration with digital health platform b.well Connected Health to support CMS’ effort to eliminate paper intake forms at doctors’ offices—also received a Certificate of Recognition from CMS for achieving general-availability excellence.

“Samsung Health gives us an opportunity to bring personalized digital health support to older adults through a platform they already use to manage their everyday health,” said Julia Hu, cofounder and President of Lark Health. “Together, we can help more people access proven prevention programs while building a foundation for ongoing, technology-supported chronic care.”

Additional information about the availability of Lark programs through Samsung Health, including applicable eligibility requirements, will be provided as each program launches.
